Am I required to claim my SBUHB Patient Record on the SBPP?

If you decide to take part in the SB Patient Portal and claim your SBUHB Patient Record, the service inviting you to take part will be able to guide you on the process of how to claim your SBUHB Patient Record. The service will also be able to answer any questions you may have before you decide whether you wish to claim your SBUHB Patient Record. 

If you choose not to claim your SBUHB Patient Record, your clinical care will not be adversely affected in any way nor be held against you in any way. There is no pressure to claim your SBUHB Patient Record.                                        

Having said that, there are benefits to having a record on the SB Patient Portal, including the opportunity to access some of your health information from home, to share that information with anyone you choose to (you may choose to share some with one person, a different section of your information with someone else, and all of it with a 3rd person for example), and the ability to communicate directly with your clinical team. 

The choice is yours as to whether or not you claim your SBUHB Patient Record on the SB Patient Portal, and you can change your mind at any time by emailing the project team – SBU.PKBProject@wales.nhs.uk. If you later remove your claim on your SBUHB Patient Record held on the SB Patient Portal, your access to this information will be removed.
