On receipt of a cardiac rehabilitation referral from the hospital you were discharged from, we will send you an information leaflet about our service and a letter inviting you to contact us and arrange an appointment with one of our Cardiac Rehabilitation Nurses. This appointment will provide you with information on your condition, the treatment you have received and the opportunity to discuss the various cardiac rehabilitation options available to support you in your recovery. The appointment can be face to face, via telephone, or virtual.
