Healthy lifestyle
All Wales Diabetes Prevention Programme
Patients who have been identified as being at risk of developing type 2 diabetes following a blood test will be offered a 30-minute intervention with a specially trained dietetic support worker.
The intervention discusses topics such as physical activity, healthy eating and promotes other lifestyle changes such as smoking cessation and alcohol reduction.
Patients are encouraged to follow the lifestyle changes discussed, with the aim of preventing or reducing the risk of developing the condition.
Newid drug and alcohol support
Newid is the single point of access for people affected by drugs and alcohol in Swansea and Neath Port Talbot.
It delivers a range of support services in a variety of settings. To be able to fully understand each person's needs, it is best to get in touch with the experienced staff members.
Newid offers open access engagement through Barod and Adferiad (formally WCADA) across Swansea and Neath Port Talbot. This means you can walk into services with no appointment required.

Social prescriber
Social prescribing is designed to support people with a wide range of social, emotional or practical needs, and many schemes are focussed on improving mental health and physical well-being.
Social prescribing schemes can involve a variety of activities which are typically provided by voluntary and community sector organisations.
The cluster's social prescriber is: Ingrid Nicholls and Nicola Rundle.

Support to stop smoking
Help Me Quit
The Help Me Quit service offers 12 weeks of free behavioural and emotional support through individual or group meetings.
Patients can access weekly sessions, either over the phone or face-to-face, where discussions involve why they smoke, behavioural changes and stresses.
It offers free confidential and non-judgemental support from a friendly stop smoking expert.
