District nursing - more information

Who do we help?

  • Patients who have a nursing need that requires assessment and ongoing care managed by a district nurse.
  • Patients who are able to give informed consent to assessment and intervention or where mental capacity is lacking via a best interest decision.
  • Patients who are 18 years of age or over.
  • Patients who are housebound in accordance with the Welsh Government definition for flu vaccination: In this instance the definition of housebound is those individuals whose medical and/or psychological condition would deteriorate adversely if they left their own home environment. (Department of Health 2010.)

 

What should I expect?

  • We will carry out a full holistic nursing assessment.
  • An appropriate plan of care will be developed and agreed in collaboration with you.
  • We will provide education and guidance to promote independence, self care and recovery.
  • We will refer onto other services/professionals with your consent if required.
  • We will work in partnership with the district nurses and other health professionals to improve health and wellbeing.

 

What does the district nursing service do?

  • Palliative and end-of-life care.
  • The administration of prescribed medication such as subcutaneous, intramuscular and intravenous.
  • Wound management.
  • Continence, including catheter management and bowel management.
  • Blood test services for housebound patients.
  • Care of enteral tubes.
  • PIC/IVs
  • Education and health promotion.
  • Diabetes.
  • Drains, such as breast drains and rocket drains.
