It can also provide you with a library of information resources like websites, leaflets, audio files and videos provided by your clinicians. You can also upload your own details, for example diagnoses, allergies, medications, symptoms, and measurements such as pulse and blood pressure readings.
The Swansea Bay Patient Portal offers you a great range of opportunities to self-monitor your condition, and in many cases you can also *direct message health professionals involved in your care. You can also share some or all of your health information with family members, carers or other health care professionals who need to see it.
