Digital Health Assessments
Digital (online) health assessments offer you the opportunity to to let your health professionals know what matters to you. By answering a few simple questions about your health and wellbeing, you can help aid the clinical decisions being made about your care.
By completing your digital health assessment, you are helping to inform decisions around your care and also help us, as providers, understand how satisfied you are with your treatment and experiences.
Swansea Bay encourages input from patients in their health decisions as much as possible. The aim is for you to receive care that is more tailored to meet your needs.
Digital Health Assessments can also benefit health board teams, including using the data collected from all patients to identify service improvements.
