Audiology clinic

Patients with hearing problems, tinnitus or problematic wax can now phone their surgery’s telephone triage system and book directly to see one of the Primary Care Audiology teams at designated clinics.

The service covers everything from a simple hearing test to assessing a sudden onset of hearing loss, which is classed as a medical emergency.

The cluster's audiology clinic is based at Penclawdd Health Centre.

Virtual ward

Each cluster has its own virtual ward, made up of a multidisciplinary team comprising health and care professionals, such as doctors, nurses, pharmacists and therapists, who discuss how to plan and manage each patient’s care.

Virtual wards provide care and support in the community to people with complex health and social needs.

Rather than a ward made up of hospital beds, the patients’ own beds become part of a virtual ward, meaning they still receive the same level of care but in the comfort of their homes.

The aim of the virtual ward is to keep patients well and healthy and home and prevent unnecessary hospital admissions, but also helping patients safely return home sooner if they have been admitted to hospital.

Dementia Hwb

The Dementia Hwb in Swansea is a unique information centre located at the site of the former Thornton’s shop in The Quadrant in Swansea.

It is staffed by a mix of volunteers from Dementia Friendly Swansea plus individuals and professionals from local organisations who support those living with dementia and their loved ones.

There are now mobile hwbs based in the community, with venues in the Llwchwr Cluster based at Gorseinon Library, Canolyfan y Bont in Pontarddulais and Felindre Lliw Reservoir café.

Social prescriber

Social prescribing is designed to support people with a wide range of social, emotional or practical needs, and many schemes are focussed on improving mental health and physical well-being.

Social prescribing schemes can involve a variety of activities which are typically provided by voluntary and community sector organisations.

Marie Curie dementia helper service

The service can offer you regular support from a trained volunteer including companionship and emotional support, practical help, a break for family and carers, and information on further support.

Action for Elders

A charity that aims to make older peoples’ lives better by offering local support with physical, mental and social programmes. 

Within this programme there is a focus on digital inclusion, mobility support, stress and anxiety, immune system stimulation, nutrition and the building of a new virtual community to enhance social interaction.

Swansea Carers Centre

Provides a range of support services and advice and guidance including benefit advice, counselling service, training, support for parent carers and those caring for someone with dementia, and volunteering opportunities to all carers in Swansea.

Swansea Dementia & Carer Project

The Swansea Dementia and Carer Project supports people living with Dementia and their Carers throughout Swansea.

The overall aim of the project is to provide people living with dementia and their carers with an opportunity to engage socially in the community, access community resources with the support of a volunteer, reduce isolation for the person living with dementia and carers and offer a listening ear to those we support.

Age Cymru dementia advocacy

The independent dementia advocacy project enables people living with dementia to access the services and support needed and to have a voice in decisions being made.

The advocacy on offer will be independent of any other service people living with dementia use. This means the person with dementia will be at the heart of the decision making process, and can be supported and represented without any conflict of interest.

Sight Life

Sight Life is a local sight loss support group with groups held in:

Gorseinon VIP Group: Gorseinon Institute, Lime Street, Gorseinon, Swansea, SA4 4EE between 10am and 12pm on the first Wednesday of the month.

Pontarddulais VIP Group: Bont Elim Community Church, Alltiago Road, Pontarddulais, SA4 8HU between 10.30am and 12pm on the third Thursday of the month.

Swansea Macular Society Support Group

The group meets bi-monthly (January, March, May, July, September, November) on the first Thursday between 11.30am and 12.30pm at the Grand Theatre Conservatory in Swansea.
