All Wales Diabetes Prevention Programme

Patients who have been identified as being at risk of developing type 2 diabetes following a blood test will be offered a 30-minute intervention with a specially trained dietetic support worker.

The intervention discusses topics such as physical activity, healthy eating and promotes other lifestyle changes such as smoking cessation and alcohol reduction.

Patients are encouraged to follow the lifestyle changes discussed, with the aim of preventing or reducing the risk of developing the condition.

Fit Jacks

Run by the Swansea City AFC Foundation, Fit Jacks is a free 12-week health and wellbeing programme, which combines fitness sessions with advice about healthy living.

It launched within the Llwchwr Cluster in 2024, with the next group sessions starting on April 24th:

Wednesdays between 10am and 12pm at Penclawdd Community Centre.

Nutrition Skills for Life is a Wales-wide programme designed to equip people across Wales to have the skills, opportunity, and confidence to access healthy, affordable, and sustainable food for themselves, their families, and their communities.

National Exercise Referral Scheme

The scheme is aimed at encouraging individuals who are currently inactive in participating in sport as part of a preventative or rehabilitation programme.

The exercise referral scheme offered by Swansea Council is part of the national exercise referral program (funded by Welsh Government).

The scheme works in partnership with health professionals in Swansea including local GPs, practice nurses, dieticians, physiotherapists, diabetic nurses, psychologists and occupational therapists.

Clients who are referred onto the scheme will get a chance to take part in a range of activities which include chair based sessions, fitness classes, Nordic walking and gym sessions. These classes last up to an hour (approximately) and are delivered in small groups of people.

Instructor-led group walks and walking groups

There are a number of led-walks and walking groups that take place around Swansea on a weekly basis - allowing for the chance to take part in regular exercise as well as enjoying the social aspects of the walk.

Here are some listed on the Swansea Council website:

Monday: 10.30am - 11.30am Gorseinon Development Centre ('Walk and Talk')

11am: Penllergare Woods dog walk. Meet in the car park and have coffee afterwards. Monday walks are off road, covering some challenging terrain and can last in excess of 2 hours.

Tuesday: 10am: Coed Bach Park, Pontarddulais. Meet at the tennis courts for a walk around the woods and playing fields followed by coffee at Pontarddulais Rugby Club.

10.15am: Gowerton Library Walking Group. The group go for a different walk every week.

Wednesday: 10am: Gorseinon Library Walking Group. The group often go by bus (bring bus pass) to various places.

10am: Elba Playing Fields, Gowerton. Leaders Donna Kendall (LAC) and Councillor Susan Jones.

11am: Loughor Health Walk. Meet at 10.50am in the Loughor Foreshore Car Park (near the play area) - free parking. Walk for an hour so followed by coffee and maybe lunch from The Mint Pod. Leader Councillor Andrew Thomas.

Thursday: 1pm: Gorseinon Wellbeing walk. North or south on the Gower Way and the new path from Kingsbridge to Gowerton, and sometimes further afield. Meet at Aldi car park. Leader Glenn Lewis.

Active Older Adults

Swansea Council's sport and health team are part of a campaign across Wales to engage older adults in sport and physical activity, working in partnership with Freedom Leisure to develop a fun, varied programme of activity across Swansea.

Selected classes are £3 and include circuits, aerobics, fitness training and aqua aerobics. While the programme also includes walking sports, coffee mornings and multi sports sessions such as short tennis and table tennis sessions.

Persistent Pain Service

The service, working with Medicines Management and with the support of GPs, has developed an education session in which persistent pain self-management strategies are shared, and support available from the service, including one-to-one therapies, medication reviews, injection interventions and pain management programmes are offered.

Referrals are made by GPs and other healthcare professionals, with the sessions provided by Persistent Pain Service staff within the Cluster.

Walking football

Walking football is an excellent way of staying physically fit and healthy. Also, if you’re recovering from an injury or operation – it is a great way of re-introducing your body to movement.

Weekly sessions hosted by Swansea City AFC Foundation provide a great opportunity to play football and socialise with others in a pressure-free, relaxed environment.

Disability sport

There are lots of opportunities for people with a disability to take part in sport across Swansea.

Pontarddulais Junior School hosts a pan-disability judo club on Mondays between 6-7pm.

Us Girls Camps

Swansea Council's Us Girls Camps are for girls aged eight to 14 and give the opportunity to try out a range of different sports and activities in a fun, friendly and non-judgmental environment across three local leisure centre sites in Swansea; Penlan, Penyrheol and Cefn Hengoed during school holidays.

Sports and activities on offer at our camps include gymnastics, rugby, self-defence, swimming, dance, mountain boarding, yoga and more.

Movement for Health class

It is a fun class aimed to help improve balance, strength, agility and flexibility to support longevity and prevent falls.

The class costs £3 and is held on a Tuesday between 10.30am and 12.30pm at The New Lodge, Alexandra Road, in Gorseinon.

Chair based exercise class

Held every Thursday between 2 and 3pm at Mechanics Institute, in Pontarddulais.

The class costs £2.50 with all ages and abilities welcome. Tea, coffee and biscuits will be available after the class.

Weekly walking group

Every Wednesday between 11am and 1pm and meets at Loughor foreshore car park. The walk is suitable for all abilities, including wheelchair and mobility scooter users.
